What it is Tramadol ?

Tramadol commonly sold under different names, is a strong opioid medication that is used to cure moderate to severe pain. It is sold under names like ultram, ultracet and other names lying in the same category. The immediate release form of the drug begins to affect the body in 30 minutes. It works in two steps. In the first step it works by reacting with the opioid receptors in the brain and then hampers the intake of serotonin and norepinephrine. Serious side effects of this medicine include risk of serotonin increase and addiction to this drug. Common side effects may include some effects that are naturally occurring in the body. These effects are noticed to be constipation, feelings of nausea and itching on the skin. People suffering from liver diseases or kidney problem may be recommended for a little change in their doses. The people with increased risk of suicide and women who are breast feeding are suggested not to take this medicine without medical supervision.

It is used for both, chronic and acute pains in the body which cause unwanted symptoms in the body. The drug comes with strong analgesic effects which last for 4-6 hour after ingestion. When compared with morphine, it was proved to be 10% potent as compared to morphine. It is proved to be affective equally in comparison with codeine.

How it works?

Tramadol is sold and commercialized with the combination of R- and S- stereo isomers. These two isomers interact with each other to enhance the effects of analgesic properties. To increase the performance of tramadol it is often suggested with acetaminophen, also known as paracetamol. It is metabolized into O-desmethyltramadol which is known to be a great opioid.

Tramadol works in two mechanisms. It is mainly due to the reason that its effects are known to be positive when working in two steps. First step starts as the tramadol salt begins to act on the opioid receptors that are in the brain and are responsible for pain signals that transfer to the brain. In the second step, it works as an anti depressant in the brain, which prolongs the activity of tramadol for 4-6 hours. Ultram, ultracet, conzip are some medicines which are used as joint salts of acetaminophen and tramadol.

Dosage and administration

Tramadol is available in many forms for the convenience of patients who have different levels of pain in their body. This makes it the most used opioid medicine in the region. Dosage forms that are available to the patients are liquids, syrup form, drops, elixirs, tablets used in water, and powder form which is used by chemist to make combined medicines. Capsules are only available with the extended release form of the medicine. Injections are the most effective from of this drug as they work directly in the blood stream.

Normal dosage values for patients suffering from pain both acute and chronic are given below.

For Patients with acute pain

Immediate-release:                      50 to 100 mg orally every 4 to 6 hours as needed for pain
Maximum dose:                            400 mg per day

For patients with chronic pains

Immediate-Release:

Initial dose:                                    25 mg orally once a day; titrate in 25 mg increments every 3                     days to reach a dose of 25 mg four times a day; thereafter increase by 50 mg as tolerated every 3 days      to reach a dose of 50 mg four times a day
Maintenance dose:                       After titration, 50 to 100 mg orally as needed for pain every 4 to 6 hours
Maximum dose: 400 mg per day

Extended-Release:
Initial dose (tramadol-naive): 100 mg orally once a day; titrate upwards in 100 mg increments every 5 days as needed and as tolerated.
Maximum Dose: 300 mg orally per day

Precautions and Warnings

Tramadol comes with many precautions and warnings. Always make appropriate measures to consult the doctor once you are feeling any abnormal effects in your body. Make sure that you are not allergic with opioids and opium drugs. Consult your doctor immediately if you have any of the following symptoms.

  • Problems like asthma or other breathing problems
  • Stomach based pains and blockages in intestines
  • If you are using narcotics, alcohol or other drugs regularly.

  • Epilepsy or injuries which had impact on brain and nervous system.

There are certain medicines which can interact with the effects of tramadol and can cause serious conditions with increased risk of serotonin syndrome. Consult your physician and tell him if you are already on the medication for depression, mental problems, headaches, infections or prevention of vomiting and motion induced nausea.

Tell your doctor to make sure that you are okay with these disorders

  • History of liver or kidney diseases
  • a stomach disorder; or
  • A history of drug abuse, alcohol addiction, mental illness, or suicide attempt.

It is more likely to cause breathing problems in older adults and people who are severely ill, malnourished, or otherwise debilitated.

Side effects

It can have serious effects when you take it as abuse and in the form of over dose. It will prove to be fatal in some conditions and you will have to take immediate medical help. Prolonged use of tramadol can cause your body to fall dependent to it. Eventually you will feel withdrawal symptoms once you stop taking tramadol immediately.

  • loss of appetite

  • Swelling in throat and thyroid glands which make you feels panic and restlessness.
  • confusion or mood problems
  • low blood pressure
  • respiratory depression
  • gastric obstruction
  • hypersensitivity reaction, which could include swelling of the tongue or throat, trouble breathing, and skin rash
